RELEASE WAIVER: The Registered Child above has my permission to participate in any/all of Trinity Baptist Church's Vacation Bible School activities. He/She is in good health and has no condition which should prevent participation. It is agreed that Trinity Baptist Church, nor its volunteers or staff assume any legal liability for injuries or other losses from participation in Vacation Bible School. I understand that in some of the activities injury may occur from participation. I hereby give my consent for emergency medical care prescribed by a duly licensed doctor of medicine or dentistry. This care may be administered under whatever conditions are necessary to preserve his/her life, limb or well being of my dependent.

Photo Consent: I hereby consent that any still or electronic image and/or audio recording, in which I or my child may appear, may be used by Trinity Baptist Church. I understand that these materials are being used for promotion of Trinity Baptist Church's Vacation Bible School program. The images and/or recordings may be used to support recruitment, fundraising, evangelization and other communication efforts.

I release the staff and volunteers and I understand and agree that the use of my picture is not an invasion of privacy. Neither I, nor anyone claiming to be speaking on my behalf, will later object to the Church's use of this/these photographs.
